When Rachel says this she is aware that Aunt Lorretta is dying. This quote can be taken in a literal way, or figurative way. In the literal way shutting or opening a door happens quick, in an instant, but people can die slowly, like Aunt Lorretta. Figuratively, people dying does not create new opportunities, as in opening a door.
